You must appeal and complete an evaluation for the State of Michigan License Appeal Board. The evaluation must include:

-The DAAD evaluation form from the state of Michigan and it must be filled out with detail to support claim of sobriety

-MAST: The Michigan Alcohol Screening Test measures past issues with alcohol and your acceptance into alcohol use

-Inform you of supporting letters as well as other documentation that is essential to DAAD

-Review your Driving Record and treatment records that were completed

-Discuss AA involvement/other support group(s) that maintains the recovery process and knowledge of the 12 Step process to recovery

-Referral to a lab for 10 Panel Drug Screen as required by the state

-Discuss the necessity of being able to support all claims of sobriety and documentation that you provided

-Referral to reputable attorney to represent you if requested
