*How do I schedule an appointment to see a therapist or psycharist at KaraLee? You can schedule an appointment for you or your child(ren) at KaraLee & Associates, P.C. by calling us at 734-451-3440. The receptionist will ask who will be seeing the therapist, your address, phone number, date of birth, what kind of insurance you have, the policy number, group number, and what time of day you have available. In addition, the receptionist will ask if you prefer a male or a female therapist, and what general issues you cannot seem to overcome. You or your child will be set up with a therapist that seems to best suit your needs, and the therapist will contact you within 24 hours to schedule your first appointment, based on their availability for the week. Most therapists are very flexible and accommodating.
*What do I need to bring with me to my first appointment? Each patient being seen needs to have their own paperwork completed (forms can be found through the PDF link above). Parents or guardians of patients under 18 years of age should fill out the Child Intake Forms, and patients above 18 years of age should fill out the Adult Intake Forms. New patients should bring their license, as well as their primary insurance card. United Health Care and United Behavioral Health require prior authorizations for the first appointment. New patients should also bring in a list of medications they are currently prescribed by their physician.
*How are payments collected? Co-pays are determined by your insurance provider and are expected at the end of each session. Monthly statements are also available through our billing service, Health Care Connect. If you have any questions about your account, please contact them at (734) 207-5226.
*How do I cancel or reschedule my appointment? Please call your therapist or the office at least 24 hours before the scheduled appointment to cancel so that other appointment times can be made.
